For exceptionally fit clients who enjoy particularly challenging physical adventures, Costa Rica's Mount Chirripo is one choice destination, Corcovado National Park another.  "Moonlight Over The Mountain" is a three-night guided Mount Chirripo trek for experienced hikers only and porters available for an extra fee.
